Here, you will find the complete forms to start therapy. Please print and fill out the Intake Packet and Privacy Notice prior to your first session. If you have any questions about these forms, we can discuss your questions in the first session. Other forms included are for later use by patients or for patients reference.
Credit Card Payment
Dr. Jerome uses the secure payment processing services of ProfessionalCharges.com to handle online Debit and Credit Card payments. Payments by credit card should be made the same day as your scheduled session.
To Make an Online Payment:

Enter your name as it appears on your card.
Insert my License Number 019645 in the space for the Provider’s License Number.
Complete the form and click on the Submit Payment button at the bottom.
Print yourself a copy of the receipt that will appear on screen.
